Abstract

Reumatoidni artritis relativno je česta autoimuna bolest koja primarno zahvaća zglobove. Etiologija bolesti je nerazjašnjena, ali su poznati brojni faktori rizika. Citrulirani proteini i odgovor imunološkog sustava proizvodnjom protutijela protiv citruliranih proteina (eng. anticitrullinated protein antibodies, dalje: ACPA) povezuje se s prekliničkom fazom bolesti. Simptomi bolesti su kod pacijenata relativno slični, ali su mehanizmi progresije bolesti neujednačeni. To je slučaj i kod upale... More sinovije gdje se kod pacijenata postoje razlike u tome koji su citokini i koje imunosne stanice prisutne u tkivu zgloba. Kod osoba s reumatoidnim artritisom razne imunološke stanice dospijevaju u tkivo zgloba gdje lučenjem raznih citokina uzrokuju upalu koja dovodi do njegovog oštećenja. Upala utječe i na ostatak tijela te se tako povećava rizik od pojave drugih bolesti. U većine bolesnika nikada ne dođe do remisije iako je to glavni cilj liječenja. Zato se i dalje istražuju novi pristupi liječenju u vidu blokiranja signalnih puteva transkripcijskih faktora povezanih s reumatoidnim artritisom, stanične terapije matičnim stanicama i kronoterapije. Abstract (english)

Rheumatoid arthritis is a relatively common autoimmune disease that primarily affects the joints. The etiology of the disease is unclear, but there are many known risk factors. Citrullinated proteins and the immune system's response with the production of ACPAs (anti-citrullinated protein antibodies) are associated with the preclinical phase of the disease. The symptoms of the disease are relatively similar among patients, but the mechanisms of disease progression are uneven. This is also... More the case with synovial inflammation, where patients show differences in which cytokines and immune cells are present in the joint tissue. In people with rheumatoid arthritis, various immune cells infiltrate the joint tissue, where they secrete different cytokines that cause inflammation, leading to tissue damage. The inflammation also affects the rest of the body, increasing the risk of developing other diseases. Most patients never achieve remission, although this is the main goal of treatment. Therefore, new treatment approaches continue to be actively researched, such as blocking signaling pathways of transcription factors associated with rheumatoid arthritis, stem cell therapy, and chronotherapy.
