Abstract

Razlikujemo četiri do pet glavnih podtipova duktalnog invazivnog karcinoma dojke koji pokazuju značajne razlike u incidenciji, preživljenju i odgovoru na terapiju. Trostruko negativni karcinomi dojke (TNBC) ne izražavaju niti jedan od hormonskih receptora (ER- / PR-/ HER2-). PD-L1 (engl. Programmed Death-Ligand 1) je ligand programirane stanične smrti, negativni regulator imunosti koji može zaštititi tumore od citotoksičnih T stanica tako da ometa ciklus imunološkog sustava unutar limfnih čvorova i unutar mikrookruženja tumora. Cilj istraživanja bio je utvrditi povezanost izraženosti PD-L1 proteina u tumor infiltrirajućim limfocitima s dobi bolesnica te patohistoškim parametrima; veličine tumora, proliferacijskim indeksom Ki-67, proširenosti tumora u regionalne limfne čvorove i prisutnost udaljenih metastaza te s podtipom TNBC; bazalnog karaktera (ˮbasal likeˮ) ili luminalnog karaktera (ˮluminal likeˮ). Imunohistokemijskim bojenjem 28 uzoraka TNBC detektirali smo prisutnosti proteina PD-L1. Svi podatci analizirani su neparametrijskim Spearman korelacijskim testom, χ2-testom i Kendall Tau analizom. Progresija bolesti TNBC češće se pojavljivala kod bolesnica kod kojih je uočena proširenost tumora u regionalne limfne čvorove kao i kod bolesnica čija je veličina tumora bila veća od 2 cm. Prisustvo proteina PD-L1 češće je bilo uočeno u bolesnica starijih od 50 godina dok samo prisustvo proteina PD-L1 nije ovisilo o proširenosti tumora u regionalne limfne čvorove kao i postotku Ki-67.

Abstract (english)

There are four to five major ductal invasive cancer subtypes that show significant differences in incidence, survival, and response to therapy. Triple-negative breast cancers (TNBC) do not express any of the receptors (ER- / PR-/ HER2-). PD-L1 (Programmed Death-Ligand 1) is a programmed cell death ligand, a negative regulator that can protect a tumor from cytotoxic T cells by interfering with the immune system cycle within the lymph nodes and within the tumor microenvironment. The aim of this study was to determine the association of PD-L1 protein expression in TNBC with age of patients, with histopathological parameters; tumor size, Ki-67 proliferation index, tumor presence into regional lymph nodes and with the presence of distant metastasis and subtype of TNBC (basal like or luminal like) . The immunohistochemical staining of 28 samples of TNBC was used to detect the presence of PD-L1 protein. All data were analyzed with the nonparametric Spearman correlation test, χ2 test and Kendall Tau analysis. Progression of TNBC occurs in patients who have had tumor infiltration into the regional lymph nodes as well as in patients with a tumor size greater than 2 cm. The presence of PD-L1 protein was observed more frequently in patients over 50 years of age, whereas PD-L1 protein alone did not depend on tumor invasion into regional lymph nodes as well as the percentage of Ki-67.
