Abstract

Iako je hemoragijska vrućica s bubrežnim sindromom (HVBS) virusna bolest uzrokovana hantavirusma, HVBS se prvenstveno smatra imunosno posredovanom bolešću. Važnu ulogu u patogenezi HVBS-a imaju citokini koje luče stanice imunosnog sustava kao odgovor na proinflamatorne signale. Interleukin 15 (IL-15), modulacijom imunosnih stanica na razini specifične i urođene imunosti, utječe na razvoj upalnih i zaštitinih imunosnih reakcija na patogene. Cilj ovog istraživanja bio je detektirati prisutnost IL-15 u perifernoj cirkulaciji u bolesnika s HVBS-om, kao mogućeg indirektnog pokazatelja razvoja protupalnih i zaštitinih imunoreakcija. IL-15 je u serumu ispitanika detektiran korištenjem Luminex® tehnologije, upotrebom multipleks imunotesta na bazi magnetskih mikrokuglica. Koncentracija RNA hantavirusa u oboljelih od HVBS-a određena je u dva koraka - metodom klasičnog RT-PCR-a su dobiveni amplikoni koji su kvantificirani metodom PCR u stvarnom vremenu. U ovoj studiji smo detektirali niske razine IL-15 u ranoj akutnoj fazi HVBS-a, nakon čega vrijednosti vrlo brzo padaju na nemjerljivu razinu. Više vrijednosti IL-15 su zabilježene u bolesnika s težom kliničkom slikom HVBS-a te bi ovaj citokin mogao imati prognostičku vrijednost u procjeni razvoja težine HVBS-a. Pokazana je korelacija IL-15 s nekoliko kliničko-laboratorijskih parametara, dok nije bilo korelacije IL-15 s viremijom. U budućim istraživanjima, za bolje razumijevanje imunopatogeneze HVBS-a, trebalo bi istražiti ulogu IL-15 u proliferaciji i diferencijaciji NK-stanica.

Abstract (english)

Although hemorrhagic fever with renal syndrome (HFRS) is a viral disease caused by hantaviruses, it is primarily considered as an immune-mediated disease. Cytokines secreted by immune cells, as a response to proinflammatory signals, play an important role in pathogenesis of HFRS. Interleukin 15 (IL-15) affects proinflammatory and protective immune reactions to pathogens on the level of innate and adaptive immunity. The aim of this study was to detect IL-15 in the peripheral circulation of patients with HFRS, as a potential indirect indicator of proinflammatory and protective immune reactions development. For detection of IL-15 in serum of patients, we used multiplex bead-based immunoassay using Luminex® technology. The viraemia in HFRS patients was determined using RT-PCR and the amplicons were quantified by qPCR. In this study we detected low levels of IL-15 in the early acute phase of HFRS after which the values rapidly fall to undetectable levels. Higher values of IL- 15 were observed in patients with more severe HFRS suggesting prognostic value of this cytokine in assessing HFRS severity. The correlation of IL-15 with several clinical laboratory parameters is demonstrated here, while there was no correlation with the viral load. In future research, for a better understanding of the immunopathogenesis of HFRS, the role of IL-15 in the proliferation and differentiation of NK cells should be investigated.
